The day of your procedure
Arrive 60 minutes before your scheduled procedure time. Wear comfortable clothing. Bring important documents:
- A copy of Advance Directives (if you have one), which will be placed in your medical record.
- Legal documentation for Medical Power of Attorney. Without this information, your procedure may be cancelled.
- List of all medications and dosages you are currently taking. You will need this information to fill out the Surgery Center forms, if you haven’t already filled out your health history form online.
- List of any allergies to medications or food
- Your insurance card and identification.
